Parent/Guardian Participation & Media Consent Form

Thank you for allowing your child to be part of Kids Take Over TV.

Kids Take Over TV is produced by Elite Media, the production company behind the show.

This form confirms your permission for us to use footage featuring your child and, where they take part in future filming, sets out some simple permissions around their participation and care while filming with us.

This form covers Kids Take Over TV footage already recorded featuring your child, as well as future Kids Take Over TV filming they choose to participate in.

Child & Parent Details

1. Participation in Kids Take Over TV

I give permission for my child to participate in Kids Take Over TV filming and related activities organised by Elite Media.

This consent covers:

• Kids Take Over TV filming that has already taken place involving my child; and
• future Kids Take Over TV filming that my child is invited to participate in.

I understand that I will continue to be provided with details of future filming opportunities and can decide whether my child participates in each filming activity.

Signing this form does not require my child to participate in future filming.

2. Looking After Our Hosts

The safety and wellbeing of our young hosts is important to us.

When a child is participating in Kids Take Over TV without their parent or guardian remaining on site, Elite Media’s policy is that they will remain under the supervision of at least one adult who holds a current Blue Card.

Parents and guardians will be provided with filming details, including expected call and collection times where practical.

At the conclusion of filming, children will be returned to their parent or guardian, or another person authorised by their parent or guardian, unless another arrangement has been agreed beforehand.

3. First Aid and Emergencies

If my child becomes unwell or is injured while participating in Kids Take Over TV, I give permission for Elite Media, venue staff or another appropriately qualified person to provide or arrange basic first aid where reasonably required.

If urgent medical attention is required, I authorise Elite Media to contact emergency services and arrange appropriate medical assistance. Where reasonably possible, Elite Media will attempt to contact me or my nominated emergency contact as soon as possible.

For any future filming, I understand that I am responsible for letting Elite Media know beforehand about any allergy, medical condition, medication or other information that may be important to my child’s safety or participation on that particular filming day.

4. Permission to Use Footage

I give Elite Media permission to use material featuring my child as part of Kids Take Over TV, including their:

• name;
• image and likeness;
• voice;
• interviews and presenter contributions;
• photographs, video and audio recordings; and
• other material recorded as part of their participation in Kids Take Over TV.

This permission applies to all Kids Take Over TV material featuring my child that has already been recorded, including appearances across multiple episodes, segments or filming activities.

It also applies to footage recorded during any future Kids Take Over TV filming in which my child chooses to participate.

5. Where the Material May Appear

I understand that material featuring my child may be used to produce, share and promote Kids Take Over TV, including through:

• television broadcast;
• streaming and on-demand services, including CTV+;
• YouTube and other online video platforms;
• Kids Take Over TV and Elite Media websites and social media channels;
• clips, trailers and promotional material;
• publicity, media and press relating to Kids Take Over TV;
• Kids Take Over TV and Elite Media showreels and presentations; and
• other broadcasters, streaming services, distributors or platforms that may carry Kids Take Over TV in the future.

Material may be made available in Australia and internationally.

6. Editing and Production

I understand that footage featuring my child may be edited, shortened or combined with other footage, music, graphics, voiceovers or other material as part of producing and promoting Kids Take Over TV.

Elite Media will make the final editorial decisions about the program, and individual edits or uses will not ordinarily be sent to me for approval before publication.

7. Ongoing Availability

I understand that episodes, clips and promotional material featuring my child may remain available as part of Kids Take Over TV and its program archive after they are first published or broadcast.

Material that has already been broadcast, published or distributed may also continue to remain available through those platforms or archives.

8. Future Participation

I understand that this form does not commit my child to any future filming.

Elite Media will continue to contact me about filming opportunities, and I can decide whether my child participates on each occasion.

If I no longer wish my child to be considered for future Kids Take Over TV filming, I can let Elite Media know at any time.

This will not affect material that has already been filmed, published or distributed in accordance with this consent.

Parent/Guardian Confirmation

By submitting this form, I confirm that:

• I am the parent or legal guardian of the child named above and am authorised to provide this consent on their behalf;
• I have read and understood the information above; and
• I consent to my child participating in Kids Take Over TV and to the use of their recorded material as described in this form.
